Enrollment Checklist

REGISTER

Use our online form to submit your preferred session date & time. We won't assign your timeslot until we receive your registration fee, so head on down to item #2!

PAY REGISTRATION FEE

Pay $50 per family to receive your timeslot assignment. Registration fee is non-refundable and covers administrative and processing fees. Use the link below to submit your payment, then continue to item #3 and keep an eye on your email for our intake forms!

COMPLETE INTAKE FORMS

After your timeslot has been assigned, we will send you an intake form* PDF. Please fill the form out completely, sign, and scan the document back to us for review. Once you've submitted your intake forms, move on down to #4 and pay your lesson fees!

PAY LESSON FEES

Use the QR Codes below to pay in full via credit card. For payment plans, AHC payment, or need-based scholarships, please contact us at livingwateriatx@gmail.com. All lesson fees must be paid in full 30 days before session start date or your timeslot will be released.

*In the rare event that a child's medical or developmental condition is any concern to our instructors, we will request a Dr's note before proceeding. We will do our best to accommodate families, but reserve the right to refuse instruction to anyone in the event it could pose a risk to the child or instructor.

Kori was made to be a swim instructor! Our four year old was very apprehensive coming into swim lessons and on the last day said, “I want to come back again and again!” Our 2yo was having a difficult time at the beginning of lessons and the way Kori cared for her with such compassion and confidence...
